What is an Integrated Model of Care?

There is a strong connection between our minds and bodies, which can affect our overall health. Using an integrated model of care, providers of multiple disciplines coordinate services to ensure patients and clients are treated as “whole” people, often resulting in better health outcomes and increased patient satisfaction. This session will summarize the scope of practice of the three Health Aide disciplines, identify opportunities to observe/assess/intervene/treat/refer patients for issues that are potentially related to another discipline, identify best practices for integrated care, and describe and identify systems of integrated care that are working well in Alaska Tribal Health System.
